How to Register

We are pleased to welcome new patients to the Practice list. In order to register, please enquire at Reception where you will be given a purple and white registration form (GMS1 Form) and a New Patient Questionnaire to complete. You will also need to bring one proof of identity eg passport, driving licence and one proof of address eg utility bill, bank statement. If you are not registered, the primary care trust have provided a service in which you can access a GP at:
